Horizon Community Mental Health provides Medi-Cal covered mental health services that meet Los Angeles County Department of Mental Health (LACDMH) standards. We serve adults 18+ with a focus on the underserved in Service Area 1, offering both Specialty Mental Health Services (SMHS) and standard outpatient support.

Specialty Mental Health Services (SMHS)
Assessment & Diagnosis
Comprehensive mental health evaluations to determine medical necessity and service needs, following LACDMH guidelines.
Individual Therapy
Evidence-based treatment for anxiety, depression, trauma, and other conditions impacting daily functioning.
Rehabilitation Services
Skill-building interventions to support independent living, coping, and community integration.
Targeted Case Management
Linking clients to medical, housing, and social resources to address barriers to care.
Collateral Services
Working with family and significant supports to strengthen treatment outcomes, as permitted by the client.
Crisis Intervention
Immediate, short-term interventions to stabilize urgent mental health needs.
Outpatient Support
Psychoeducation
Education on mental health conditions, treatment, and coping strategies.
Wellness & Prevention
Tools for stress management, relapse prevention, and ongoing self-care.
Care Coordination
Collaboration with healthcare providers and community partners to ensure integrated, whole-person care.
Compliance & Access
- Coverage: Medi-Cal; medical necessity confirmed per LACDMH standards.
- Documentation: Treatment plans, progress notes, and outcome measures maintained per county and DHCS requirements.
- Access: Telehealth and in-person services; language assistance available.
- Rights: Clients receive Notice of Privacy Practices and Grievance/Appeal information.
