Horizon Community Mental Health provides Medi-Cal covered mental health services for adults 18+ in Lancaster, Palmdale, and the greater Antelope Valley. Services include outpatient support and Specialty Mental Health Services when medically necessary.

Targeted Case Management
Linkage to medical, housing, social, and community resources that may support recovery and stability.
Collateral Services
Coordination with family members or other supports, when authorized, to strengthen treatment and outcomes.
Crisis Intervention
Short-term support to address urgent mental health needs and help restore stability when clinically appropriate.

Horizon Community Mental Health supports adults experiencing anxiety, depression, stress, trauma, grief, life transitions, emotional overwhelm, social isolation, adjustment difficulties, and other mental health concerns affecting daily functioning and wellbeing.
